If a drunk customer from a bar drives home and crashes, is it also the fault of the bartender?

and if so, why would it be? how are bartenders supposed to monitor every single customer on what they drink and when they leave? If a drunk customer from a bar drives home and crashes, is it also the fault of the bartender?
Depends on the state
In America, yes. It could vary a little by state. You are supposed to assess how much you have served the person, and how much it is affecting them. You are supposed to mentally keep track, and assess their speech and composure. It is kind of crazy how responsible the bartender is.

I believe it is just an extra attempt to limit drunk driving on the roads, which is fair, but that doesn't do much good to the bartender who has someone sneak in alcohol into the bathroom and then order one or two drinks and get hammered.
So, yes the bartender can be held responsible for drunk drivers who hurt someone. If a drunk customer from a bar drives home and crashes, is it also the fault of the bartender?
Yes, in many states there are "dram shop" laws, passing liability for actions of drunks on to those who were paid to get them drunk.

These stupid laws should be repealed and replaced with statutes that allow anyone aware of a drunk to stop that person from driving drunk by using any amount of force necessary, including lethal force, in order to defend the innocent victims who happen to be sharing the road with drunks.

I'd rather have a few drunks get a well-deserved beating by concerned citizens than read about another roadway tragedy caused by a drunk driver who nobody tried to stop.
Depends on the state and the situation; usually it's if the bartender continued to serve someone who was obviously intoxicated, such as in this case:
